Request For Return of Parental Consent Forms
Permission Request to Take & Use Images of Swimmers
Parents of swimmers under the age of eighteen may recall that a new form was issued this year as part of the documentation sent out with the annual fee notifications. The form requested parental permission for the taking and use of images of swimmers who are under eighteen years of age. Parents were given the option to either consent or refuse the taking and use of pictures and images for a number of defined uses (newspaper reports, club noticeboard, website, training purposes etc).
Whilst a number of these forms have been returned there are still quite a number that remain outstanding. Please can all parents/guardians of swimmers under eighteen years old, who have yet to do so, take a few minutes to complete and return their forms.
